Case Study 1 — Amusement Park Ride Roof Collapse

Problem:

Installation of new wooden roof trusses were being installed by the contractor when a 35 mph rain storm wind blew down the trusses.

Process:

Investigate all damaged wooden trusses, wall anchors, wooden bracing between trusses, temporary bracing and fasteners.  Review drawings and manufacturing specifications from truss supplier, architect and engineer of record.  

It was found that the truss installation contractor failed to install required wind bracing as per manufacturer’s specifications therefore allowing a small rain storm wind to blow the erected trusses over against each other which created a domino effect collapsing the truss installation.

Our Solution:

Remove all damaged trusses, purchase new and install with specified wind bracing as per manufacturer’s specifications.

Case Study 2 — Railroad ATV Accident

Problem:

A serious ATV accident occurred near a railroad crossing, and the client needed to determine whether the cause was related to roadway design, visibility, or safety measures.

Our Solution:

We conducted an on-site forensic investigation to assess the conditions surrounding the new railroad gate and roadway.

Results:

Our findings showed that the railroad company had installed an unmarked and unlit gate approximately 300 yards off the nearest roadway.


The gate was open during the day but
closed at night with no signage, lighting, or cautionary markings, making it invisible to the ATV driver.


This analysis provided clear evidence of design and safety non-conformance, establishing liability and preventing future incidents.

Case Study 3 — Church Flooding and Roof Dome Water Intrusion

Problem:

A church building suffered widespread flooding and water intrusion just six months after completion. The client needed to determine the cause of sprinkler pipe failures and roof leaks that damaged interior finishes and ceiling artwork.

Our Solution:

We performed extensive investigations across the roof, attic spaces, dome structure, and wall assemblies, identifying insulation and detailing errors that led to freezing and water intrusion.

Results:

Our team found inappropriate insulation materials and improper detailing of the church dome base structure.


Through our
mediation and construction management services, we coordinated with the general contractor and subcontractors to re-insulate attic spaces using spray foam, replace defective materials, and reconstruct the dome’s exterior walls.


The project was resolved successfully —
without legal litigation — thanks to our collaborative, expert-led approach.

Case Study 4 — Foundation Failure

Problem:

A building’s foundation was cracking and separating.

Process:

Site investigation which included core borings, geological research, construction document and specifications reviews which indicated that concrete caisson depth was not sufficient to support the building load.

Solution:

Provide temporary building load support while an additional concrete caisson was installed.
